In early 2015 we welcomed Focus Health into the Covenant Care family and created Elevate Home Health. With the addition of Focus Health, we have expanded our skilled home health services to all the markets listed to the left. We also offer personal care in select markets and plan to expand to many of the areas in which Covenant Care facilities are located, from California to Ohio.

Covenant Care interdisciplinary teams work to integrate care services directly with our patients and their families to maximize outcomes and enhance healthcare transitions. Certified services are provided under the care of a supervising physician, who oversees all aspects of patient care.
